A new analysis out of Tufts University's Food is Medicine Institute makes that pitch harder to take at face value. Pooling data across 21 randomized controlled trials, researchers found non nutritive sweeteners were linked to worse blood sugar control, not just a neutral swap for sugar.
The team, based at the Friedman School of Nutrition Science and Policy, focused specifically on trials that compared sweeteners against non caloric controls like water or placebo, rather than against sugar itself. That design choice matters, because it isolates what the sweetener molecules themselves are doing in the body, separate from any calories being replaced.
Across those 21 trials, sweetener intake raised fasting insulin and HbA1c, the standard marker doctors use to track blood sugar control over the prior few months, and trended toward worse insulin sensitivity overall. Separate observational data cited in the same review also tied regular non nutritive sweetener use to higher cardiometabolic disease risk, with gut microbiome disruption proposed as a likely pathway connecting the two.
The review's own authors flag real limitations. Much of the supporting data has reverse causation risk, meaning people already at higher metabolic risk may be the ones reaching for diet products in the first place, which could make sweeteners look worse than they are. The review also lumps different sweeteners together, even though aspartame, sucralose, stevia, and monk fruit likely behave differently in the body. The authors are explicit that this is growing evidence calling for more randomized trials, not a settled cause and effect finding.
