Track your body composition, not just the scale. I have written some version of that sentence more times than I can count, and until now the honest reason behind it was mostly aesthetic. A Danish study published July 29, 2026 gives it a much more concrete one. I read through it, and here is what it actually says.
A team led by Dr. Nis Brix at Aarhus University worked with 1,058 young Danish men, recruited around their 18th birthday from the Fetal Programming of Semen Quality cohort. Instead of relying on BMI, they measured body fat directly with a bioimpedance analyser, then compared body fat percentage, BMI, and waist to height ratio to see which one best predicted semen quality, testicular size, and reproductive hormones. Men in the high body fat group, 20% to 24%, had 23% lower sperm counts than men in the normal range of 8% to 19%. Men in the very high group, 25% and above, were 15% lower. Their hormones moved too: testosterone and sex hormone binding globulin came in lower, luteinising hormone and oestradiol higher. That last part matters, because a hormonal shift alongside the sperm numbers suggests something systemic rather than a random blip in one measurement. The paper published in Human Reproduction.
Here is the part I found genuinely interesting. When the researchers compared like for like, using standard deviations so the metrics could be measured against each other fairly, one standard deviation more body fat tracked with a 13% lower sperm count. The same size jump in BMI tracked with only 8%. Body fat percentage was meaningfully the better predictor. And the effect held in men whose BMI sat squarely in the healthy range, which is the whole point. BMI cannot tell muscle from fat. It sees a number on a scale and a height, and a man who is carrying more fat than his frame suggests will sail through that screen while sitting in exactly the group this study flagged. For once, the biohacker obsession with body composition over body weight has a peer reviewed health outcome attached to it rather than a mirror.
This is one cohort, not yet replicated by anyone else, and only about 19% of the men invited actually took part, which raises real questions about who chose to show up. The pattern is also not a clean staircase: the very high body fat group came in at 15% lower, less than the 23% seen in the high group, which is not what you would expect if more fat simply meant fewer sperm, and it hints at noise in the smaller subgroups. Both affected groups still averaged above the accepted fertility reference point of 39 million sperm per ejaculate, so this is a population level shift and not evidence that any individual man here is infertile. Semen volume and concentration pointed the same direction but did not reach statistical significance, and motility, shape, and testicular size showed no clear pattern at all. So this is a sperm count and hormone story specifically, not a blanket claim that body fat ruins fertility. The authors say plainly that this needs confirming in other populations, and a follow up study across Danish and Norwegian men is already running.
Nothing here asks you to panic or overhaul anything today. What it does is quietly upgrade a habit a lot of us already have. If you own a bioimpedance scale and have been treating the body fat reading as a vanity metric next to the weight, this is a reason to flip that priority, with the caveat that home scales drift and the trend over months tells you far more than any single morning reading. If you have only ever been screened by weight and height, this is a reasonable thing to raise at your next checkup, particularly if your weight looks fine but you suspect your composition does not match it. And if you are actively trying to conceive, the useful move is a real semen analysis through your doctor rather than inference from a study about someone else. I am watching for the replication, because that is what would take this from a compelling signal to something I would actually build advice around.
The researchers used a bioimpedance analyser to sort men into three groups: normal body fat at 8% to 19%, high at 20% to 24%, and very high at 25% and above. Two things are worth holding onto. These were young men around age 18, so the cutoffs are not a universal standard for every age or body type. And bioimpedance is an estimate, convenient and repeatable but less precise than a DEXA scan, so treat the categories as useful buckets rather than exact truth.
No, and the study does not say that. BMI is still a fast, free, reasonable screen across a population, and it did show a signal here, just a weaker one. The point is narrower and more useful than throwing BMI out. BMI cannot tell the difference between muscle and fat, so a man carrying more fat than his weight suggests can pass a standard weight checkup and still sit in the group this study flagged. If one number is the only number you ever look at, that is what you are missing.
Almost certainly not on the strength of this study alone. Both higher body fat groups still averaged above the World Health Organization lower reference point of 39 million sperm per ejaculate, so this is a shift in a population average, not evidence that any individual man is infertile. Sperm motility, shape, and testicular size showed no clear pattern at all. If you are actively trying to conceive and something feels off, a semen analysis ordered through your doctor tells you about you, which is the one thing no cohort study can do.
